After serving with distinction with the Australian Imperial Force during WWII, he joined the Royal Prince Alfred Hospital (RPA) in 1947, the same year in which he established his consultant practice as a general physician and gastroenterologist. In 1948 he and William Morrow set up the first specialist gastroenterology unit in Australia at the RPA. It was notable for encouraging collaboration on recovery programs between the surgeons and other practitioners. Goulston worked closely with Vincent McGovern, pathologist the Hospital, in studying acute and chronic hepatitis, ulcerative colitis, Crohn's disease, ischaemic colitis, pancreatitis and functional disorders. For many years Goulston was active in the Royal Australasian College of Physicians, serving as Censor-in-Chief, and President from 1974 to 1976. Other organisations of which he was President include the Gastroenterology Society of Australia and the New South Wales branch of Legacy.
