In 1851 he migrated to New South Wales with his family. While in private practice in Sydney, he also held positions at Darlinghurst Gaol and the penal establishment at Cockatoo Island. He gave evidence on several occasions to Parliamentary enquiries on the running of these institutions. In 1859 he became a founding Council Member of the Australian Medical Association. When the University of Sydney established its medical school in 1856, West was appointed Examiner in Medicine, a position he held until his death.
