His early practice was in both radiotherapy and radiodiagnosis, but by the 1920s he was specialising in diagnosis. For over 40 years he was in private practice in Sydney, while also holding honorary positions in a number of Sydney's leading hospitals. He became Head of the Department of Radiology at Royal Prince Alfred Hospital, Sydney. Sear played a key role in the foundation of the Australian and New Zealand College of Radiology in 1935 and was President of its successor, the College of Radiologists (Australian and New Zealand). On retiring he gave his large collection of slides of bone diseases to the Faculty of Medicine at the University of Sydney.
