He invented a portable machine for mechanical or insufflation anaesthesia, the "Lidwill Inter-tracheal Anaesthetic Machine", which he patented in 1921 and which was used in hospitals across Australia for over 30 years. Lidwill also invented the cardiac pacemaker, based on his discovery that electricity could be used to set the pace of a diseases heart. He was the first Director of Anaesthetics at the Royal Prince Alfred Hospital, Sydney.
