For nearly 50 years he had a central role in St Vincent's Hospital, Sydney, as Neurosurgeon, Dean of the Clinical School for more than 30years, and in retirement as Chairman of the Advisory Board. The neurosurgical unit at St Vincent's has been named the Douglas Miller Department of Neurosurgery. His efforts to engage his neurosurgical colleagues in a professional organisation resulted in the formation in 1940 of the Society of Australasian Neurological Surgeons. Miller was active in the Royal Australasian College of Surgeons, being President 1957 - 1959, Chairman of the Editorial Committee for the College's journal for 16 years, and Honorary Archivist for eight years. Between 1956 and 1960 Miller made a number of visits to Southeast Asia under the Colombo Plan to give demonstrations in neurological surgery.
