He first joined the hospital in 1941 and left in 1943 for war service. He returned to St Vincent's in 1950 and even though he officially retired in 1983 he continued on as a Consultant Physician. Cahill also managed to fit in a number of other roles outside of the hospital including Clinical Instructor of Medicine at the University of Melbourne, Commonwealth medical referee for compensation cases and chief medical officer for a number of international life insurance companies. He also ran a very successful private practice until the mid 1990s and became the "physician's physician" as many of his peers specifically sought his help when they became ill.
