On returning to Melbourne, he entered partnership with leading radiologists Stanley Argyle and Herbert Hewlett, and joined the Melbourne Radiology Clinic. Macdonald was appointed Radiologist at the Women's (later Royal Women's) Hospital in 1927 and was subsequently Director of Radiology. He was also Honorary Radiologist at the Children's Hospital, Melbourne from 1933 to 1949. In 1949 he became a Foundation Fellow of the College of Radiologists of Australasia. Macdonald was a keen historian, publishing a number of papers on medical history.
