Upon his return, MacCarthy worked as a resident clinical associate at Dublin's Mater Misericordiae Hospital. This was followed by short stints in Manchester, England and in Ireland (Fethard and Clomnel). He then enrolled at the University of Brussels and graduated with a Doctor of Medicine in 1884.
