Having joined the Royal Prince Alfred Hospital in 1899 as a Junior Resident Medical Officer, he was closely associated with the Hospital until his death in 1972. He was also connected in an honorary capacity with other leading Sydney hospitals, the Royal Hospital for Women, the Royal Alexandra Hospital for Children and the Prince Henry hospital. Blackburn was a central figure in medical associations in New South Wales. A long-standing Member of the British Medical Association New South Wales Branch, he served as President from 1920 to 1921. He was a founding Member of the Association of Physicians of Australasia (the forerunner of the Australian Medical Association), and inaugural President of the Royal Australasian College of Physicians. Blackburn lectured in clinical medicine at the University of Sydney from 1913, ultimately becoming Dean of the Faculty of Medicine. A member of the University's Senate for many years, Blackburn was Chancellor from 1941 to 1964.
